Want to request a copy of your birth certificate or social security card? Are you looking to find more information on your placement history or biological parents/relatives? The Important Life Documents services can help you locate all of your most vital records.

What are Vital Records?

Vital records are documents that provide proof for major events in one’s life. For example, social security card, birth, death, and marriage certificates. All youth in out of home care should have copies of their records. To receive copies of your birth certificate, visit the Maryland Department of Health.

Before you leave foster care, make sure you have these documents for your records:

  • Letter from the Social Services Administration Director
  • Birth Certificate
  • Social Security Card
  • Maryland State Identification Card (Required) or Driver’s License (if applicable)
  • Medical Insurance Card
  • Last Known Address of Parents/Relatives
  • Placement History
  • Child Welfare History
  • Medical Records 
  • Education Records
  • Credit Report
